I like to customize the fields in the "Add New Address" form that Users are prompted to fill out in the CheckOut section. Is it possible to do that. I just want to remove some of the defaults fields and add couple of other fields. Any help would be appreciated.

The form on the attached image is what I am trying to customize. I like to remove some of those fields and replace them with few other questions.
